Job Description
AbilityPath is seeking a Helpful Early Childhood Mental Health Coordinator to support families in San Mateo County. The role focuses on connecting families with resources, providing care coordination, and coaching on mental health and social-emotional well-being. This position supports both parent and child mental health, contributing to building and maintaining a comprehensive database of services and driving community partnerships.
Requirements
- Provide warm referrals and follow-up for families with social-emotional or developmental concerns.
- Develop and maintain a database of early childhood mental health services in San Mateo County.
- Conduct case reviews and contribute to continuous quality improvement.
