Accord Care is seeking a Clinical and Services Coordinator (NDIS) to join their VIC team. The role combines clinical coordination and operational service delivery, with a focus on participant wellbeing, risk management, and service quality improvement.

Requirements

• Bachelor of Nursing or equivalent qualification
• Current registration with AHPRA as a Registered Nurse
• Minimum 2–3 years' experience in healthcare, disability, aged care, or community settings
• Experience working with complex care and high-intensity supports
• Strong understanding of clinical risk, care planning, and medication management
• Excellent documentation and communication skills
• Ability to manage competing priorities in a fast-paced environment
• Current NDIS Worker Screening Check
• Current Working with Children Check
• Current driver's licence
