Job Description
As a Licensed Practical/Vocational Nurse (LPN or LVN) on the Pediatric Transition Team, you’ll ensure children and families experience a safe, supported, and compassionate transition from hospital to home.
Requirements
- Active LPN or LVN license
- Current CPR certification
- Pediatric and/or home health experience preferred
- Trach/vent experience a plus
- Bilingual preferred (not required)
