Job Description
Angels of Care is seeking a Pediatric Transition Nurse to ensure children and families experience a safe, supported, and compassionate transition from the hospital to home. This role involves assessment, hands-on nursing, education, and mentoring, aiming to prevent readmissions and build family confidence. The team focuses on providing coverage during staffing gaps.
Requirements
- Active LPN or LVN license
- Current CPR certification
- Pediatric and/or home health experience preferred
- Trach/vent experience a plus
