Job Description
The Pediatric Transition Team at Angels of Care provides support to children and families as they transition from a hospital to home. Nurses will assess patient needs, deliver hands-on pediatric nursing, educate families, mentor team members, and be a trusted resource. This role involves ensuring safe transitions and building family confidence.
Requirements
- Active LPN or LVN license
- Current CPR certification
- Pediatric and/or home health experience preferred
- Trach/vent experience a plus
