Job Description
Angels of Care is seeking a Licensed Practical/Vocational Nurse (LPN or LVN) to join their Pediatric Transition Team. The role involves ensuring children and families a safe, supported transition from hospital to home, including providing hands-on pediatric nursing and case coordination. Nurses will act as a trusted advocate, mentor, and resource. The team ensures continuity of care and provides coverage during transitions.
Requirements
- Active LPN or LVN license
- Current CPR certification
- Pediatric and/or home health experience preferred
- Trach/vent experience a plus
