HIV Medical Case Manager

Boston Health Care for the Homeless Program is offering a Medical Case Manager position to work with a vibrant multidisciplinary HIV team to address key social determinants of health for people living with HIV and substance use disorder who are experiencing homelessness.

Requirements

• Assess new HIV patients for financial, psycho-social, housing, and other needs
• Develop comprehensive service care plan with the patient
• Work with patients to reduce barriers to full engagement in HIV care and treatment
• Identify patients who will benefit from outreach visits
• Conduct intensive outreach visits to connect with patients who are newly diagnosed with HIV and not yet in care
