Case Manager, Transitional Housing

We are seeking a Case Manager, Transitional Housing to provide case management services to individuals experiencing homelessness in order to assist them in transitioning to permanent housing. The position involves conducting assessments, benefits assistance, and connecting clients to psychiatric services, medical treatment, and substance use treatment facilities. The Case Manager will also assist clients in obtaining permanent housing, maintain record keeping, and intervene in crisis situations.

Requirements

• Bachelor’s Degree or equivalent work experience
• Two years of experience working with homeless adults and substance abuse issues
• Experience with harm reduction and motivational interviewing techniques
• Valid NY, NJ, CT, or PA driver’s license with a good driving record
• Excellent computer skills
• Excellent writing and communication skills
• Bilingual Spanish/English
