Hospice Social Worker

By The Bay Health is seeking a Social Worker to join its hospice transdisciplinary team, providing supportive services to patients and families coping with terminal illness and loss.

Requirements

• Assess patient/family social and emotional factors to determine ability to cope with terminal illness and death
• Provide individual and family counseling to patient/families in all psychosocial aspects of end-of-life care
• Assess environmental resources and obstacles to maintaining safety
