Job Description
CAN Community Health is now hiring a Medical Case Manager II to coordinate and deliver comprehensive case management services to clients, ensuring access to care and support in compliance with local, state, and federal guidelines.
Requirements
- Bachelor's degree in social work, Nursing, Public Health, or related field or 4/7 years' experience in same fields.
- Three years of HIV prevention/intervention experience
- MCM Certification for TGA, or able to obtain within 90 days
- Valid driver's license and insurance
- Ability to pass a Level I and Level II Background check
