Role Overview

We are seeking a skilled and dedicated Registered Nurse (RN) Case Manager to join our team. The Registered Nurse (RN) Case Manager will be responsible for providing comprehensive case management services to our clients, ensuring they receive the highest level of care and support.

What You Will Do

Complete new client admissions, meet with families and clients, develop initial care plans, oversee caregivers, ensure compliance with state regulations and agency policies, and maintain accurate documentation.
