The residents Case Manager is a mid-level social services and advocacy position responsible for proactively working with residents of Jan Peek House, community service providers, including educational and employment resources, DSS, housing programs and landlords to assist residents with obtaining stable housing and services leading to independence and long-term stability.
Requirements
- Conduct timely intake and assessment of new residents
- Meet with residents to develop, implement, and review individualized service plans
- Provide support services related to independent living skills including budgeting, connection to community resources, and access to support, employment and health services
- Provide advocacy for residents for services and benefits
- Assist residents with securing stable housing
- Ensure Emergency Service (ES) clients receive timely, comprehensive assessment of immediate needs, risks, and service requirements
- Maintain files “audit ready”
- Work across programs when needed
- Participate in staff meetings, conferences, and organizational events
- Provide data and reports
