Job Description
The Health Home Registered Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through case finding, health teaching and counseling. The nurse will provide interventions supportive to or restorative of life and well-being, and execute medical regimens prescribed by a physician, PNP or another licensed health care provider.
Requirements
- Associates Degree in Nursing or equivalent
- License and current registration to practice as a Registered Nurse in New York State
- Valid NYS Driver License required
- 1 year post RN licensure clinical nursing experience working with individuals with mental disabilities preferred
- Bilingual Spanish speaking preferred
- Clinical Experience preferred
