Patient Navigator RN

The Patient Navigator RN will assist patients to understand their diagnosis, coordinate their treatment plans, and communicate with providers and staff. Using a combination of communication strategies, resources, and tools, the Navigator will assist patients to anticipate, identify, and achieve effective steps through the various treatment options available.

Requirements

• Guides patients and families through their healthcare experience
• Follows patients from date of diagnosis through the completion of the care-path
• Reviews available information obtained by other health care team members
• Communicates problems with workflow
• Collaborates with surgical scheduling department
• Maintains professional and technical knowledge
• Performs other position related duties as assigned.
