Home Health Licensed Practical Nurse

As a CenterWell Home Health nurse, provide personalized care to patients in their homes, working with a dedicated team of physicians and clinicians to manage care plans and help patients regain independence.

Requirements

• Current nursing license in the practicing state
• Valid drivers license, auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting
