Case Manager RN-Lynchburg Home Heatlh-FT-Saturday, Sunday & Mondays

The Home Health RN Case Manager will be responsible for planning, coordinating, and managing the care provided to assigned patients. This role involves collaborating with the interdisciplinary team to create individualized care plans, promoting a safe environment, and contributing to a healthy workplace culture, in accordance with ANA and VA nursing regulations.
