Job Description
Reporting to the Manager of HIV Services, the Medical Case Manager provides medical and social services case management to adult clients with HIV/AIDS in the Codman community, providing high-level advocacy and managing biannual referrals for state and Federal medication assistance programs.
Requirements
- Associates degree or minimum of 2-years progressive professional experience
- Knowledge and experience with HIV services
- Awareness of substance abuse issues
- Ability to work within a wide community and social service network
- Familiarity with Electronic Medical Records (EMRs) and Electronic Health Records (EHR) systems
- Ability to speak Haitian Creole
