Case Manager - Riverfront Lofts - 10903

The Case Manager will work collaboratively with case managers, supervisors, integrated health team members, and community-based partners to promote long-term solutions for families, children, and individuals experiencing homelessness or who have formerly experienced homelessness.

Requirements

• High School Diploma or GED required
• 1-2 years of experience working with a diverse population who are experiencing mental health challenges, particularly those individuals diagnosed with serious and persistent mental illness (SPMI) preferred
• Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders preferred
• Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred
• Proficiency in Microsoft Office Suite
• Knowledge of local housing resources, benefits programs, and social service agencies preferred
• Bilingual or multilingual skills preferred
