Job Description
CHC/SEK is seeking a Substance Abuse Counselor to support the care team, ensuring patients are connected to needed resources. The role involves assisting the team in integrating care, providing case management, counseling, and utilizing diagnostic and behavioral assessment techniques. Focus is on positive interaction, goal setting, and collaborative treatment planning.
Requirements
- Is available for immediate consultations with members of the Care Team per the SBIRT protocol.
- Administers drug and alcohol screenings and facilitates clinical assessment.
- Coordinates client services through assessments and discharge/aftercare.
