RN Case Manager, Skilled Nursing

The RN Case Manager provides support and clinical expertise across the continuum of care, collaborating with physicians and the interdisciplinary team to coordinate patient care, enhance clinical outcomes, and manage the cost and efficiency of care delivery.

Requirements

• High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate
• 1 year of nursing experience in the assigned care setting
• RN - Current and valid Registered Nurse license to work within the state of California
• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)
