Care Coordinator - WISe

Care Coordinators work with youth and their families to develop and implement individualized, cross-system care plans that work toward the family’s long-term goals.

Requirements

• Maintains CANS certification throughout employment.
• Conducts CANS screening and CANS full assessments.
• Conducts and/or provides information to ensure full CANS assessments are completed every 90 days for assigned Wraparound with Intensive Services (WISe) children/youth on active caseload.
