Clinical Pharmacy Specialist (Renal Transplant Nephrology)

The Clinical Pharmacy Specialist will provide expert pharmaceutical care to patients with renal transplant or nephrological conditions, both in the ambulatory and inpatient settings, and develop individualized treatment plans.

Requirements

• PharmD from an ACPE accredited school of pharmacy is required
• Completion of a PGY1 and applicable PGY2 specialty residency or a minimum of 5 years of clinical experience required
• TX Pharmacist license or eligible
• Preceptor license required (must be obtained within 90 days)
• CPR – BLS required
• BPS certification required within 1 year of employment
