Job Description
The ECM Care Manager – Licensed Vocational Nurse (LVN) facilitates patient continuity of care and supports individualized care plans. The LVN coordinates patient needs, engages patients in healthy behavior changes, and collaborates with the care team to manage patient risk factors. They participate in quality improvement and offer advocate services.
Requirements
- Implement and support individualized patient care plans
- Address patient risk factors and/or obstacles to care
- Facilitate engagement between the PCP and patient
- Demonstrated ability to motivate others to change
- Participate in required care coordination conferences
- Maintain a predetermined patient caseload
- Exercise sound clinical judgment
- Seek advice and sign off from a Registered Nurse when necessary
- Take initiative in finding solutions to problems
- Work as a patient advocate
- Performs miscellaneous job-related duties
- Document clinical data accurately
- Adapt to new technologies and software
- Ability to navigate and work in high-volume environments
