Care Manager RN

Role Overview

The Care Manager RN is a key member of the Special Needs Plan (SNP) care team, responsible for coordinating care for members with multiple chronic medical and behavioral health conditions, as well as social determinants of health (SDoH) needs. This role involves conducting comprehensive assessments, addressing SDoH challenges, and developing individualized care plans.

What You Will Do

Conduct telephonic engagement with members, compile clinical information, develop care plans, provide disease management education, and coordinate care with the interdisciplinary team.

Requirements

• Active and unrestricted Registered Nurse (RN) licensure in the state of MI or compact licensure in state of residence
• Proficient in Microsoft Office Suite
• Minimum 3+ years of nursing experience
• Minimum 2+ years of case management, discharge planning, and/or home healthcare coordination experience
