Job Description
We are hiring a Community Health Worker to provide comprehensive and coordinated interventions to address the social determinants of health. As a key member of the healthcare team, the CHW will facilitate access to care and services, provide education and advocacy, and support patients in navigating the healthcare and social service systems.
Requirements
- Basic knowledge of Microsoft Office
