HIV Medical Case Manager (Hybrid)

Erie Family Health Centers is seeking a HIV Medical Case Manager to join their Lending Hands for Life team. The successful candidate will work with providers and staff to coordinate medical services, access to care, and other services for HIV+ clients. The role involves managing a caseload of higher-need clients, providing medical case management services, and referring clients to appropriate services.

Requirements

• Bachelor's degree in Social Work or related field with licensure in mental health counseling
• Associate's degree with 4 years of case management experience
• 2 years of experience in community health or with a similar patient population
• Familiarity with issues affecting individuals living with HIV
• Verbal and written communication skills
• Intermediate proficiency in Microsoft Office
• Bilingual fluency (English/Spanish)
