PrEP/ HCV Patient Navigator (Hybrid)

Join Erie Family Health Centers as a PrEP/HCV Patient Navigator and contribute to providing high-quality, affordable care to diverse patients. You will build individual and community capacity through outreach, education, counseling, and advocacy, working to reengage people who are HIV+, HepC+, or at risk in medical services.

Requirements

• High School diploma or equivalent required
• A minimum two years of work experience in health, social or community services preferred
• Bilingual in fluency (English/Spanish) strongly preferred
• CPR certification required
• Verbal and written communication skills, including high level of comfort presenting to groups
• Microsoft Office computer skills desired
