Form Health is a virtual obesity medicine clinic delivering multi-disciplinary evidence-based obesity treatment through telemedicine. We are seeking Physicians specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team.
Requirements
- Board certification in Endocrinology by the American Board of Internal Medicine (ABIM)
- Highly preferred: Certification by the American Board of Obesity Medicine (ABOM) or demonstrated extensive clinical experience in obesity medicine and pharmacotherapy
- Minimum of 5 years as an attending physician in endocrinology
- Demonstrated competency and comfort in managing patients with complex co-morbidities, including CKD, ESRD, and CAD/CV conditions
- Expertise in advanced diabetes management, including the use of CGMs, insulin pumps, and complex insulin regimens
- Strong interest in mentoring, teaching, and collaborating with Endocrine-Specialized APPs and Registered Dietitians specializing in diabetes
- Demonstrated experience working with APPs, medical residents, or trainees in a clinical academic setting
- Excellent communication and interpersonal skills
- Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for both endocrine disorders and obesity
