We provide personalized, coordinated clinical and social care as well as health plan coverage through the Program of All-Inclusive Care for the Elderly (“PACE”) in collaboration with our leading healthcare partners, including Kaiser Permanente.

Requirements

• 3+ years of experience in a healthcare-related field
• Current licensure in good standing (e.g. RN, PT, OT, MA)
• Ability to work in a fast-paced environment and adapt to changing priorities
