Job Description
Harmony Hospice Care is seeking a dependable, compassionate, and organized RN Case Manager to join our team, to build meaningful relationships, support patients and families, and provide care with professionalism, consistency, and compassion.
Requirements
- Manage a structured caseload of up to 18 patients
- Serve patients in San Antonio and surrounding areas just outside city limits
- Build strong, supportive relationships with patients and families
- Collaborate with a team that values communication, accountability, and compassion
