Health Navigator-PrEP

Health Navigators work within the Medical Suite, providing health education, supporting patient needs, and linking patients to appropriate resources. They collaborate with staff and teams to create personalized care plans, focusing on chronic medical conditions and healthcare navigation.

Requirements

• High school diploma/GED/or equivalent of 2 years work experience in social service setting required
• Bachelors in social services or health related field preferred
• Knowledge of HIV, STIs, PrEP and Hepatitis C highly preferred
• Experience working in health care and navigating health insurance is highly beneficial
• Experience with Electronic Health Records preferred
• At least 1 year previous experience working with substance users and knowledge around substance use and abuse required
• Phlebotomy training preferred
• Bilingual (English/Spanish) preferred
• Valid Illinois Driver’s License and eligibility for agency insurance coverage is preferred
• Understanding of HIPPAA, 42CFR, AIDS Confidentiality Act.
• Previous case management experience beneficial
• Previous counseling and testing experience beneficial
