Job Description
Ontario Health atHome is seeking a Care Coordinator to assess and determine patient care needs, determine eligibility for services, and provide access and referrals to community resources. This role involves collaboration with patients, caregivers, and health care practitioners to provide connected, accessible, and patient-centered care. The position offers professional growth and work-life balance support within a supportive team environment.
Requirements
- Membership, in good standing, with the applicable regulatory body
- Recent experience in community health or a related field
- Knowledge of the health care delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving and decision-making skills
- Effective time management, prioritization, and organizational skills
- A valid driver’s license and access to a reliable vehicle
- Proficient in a Windows environment
- Advanced oral and written proficiency in English and French
- COVID-19 vaccination status required
