Job Description
Healthfirst is seeking a Field Assessment RN to conduct in-person assessments throughout Brooklyn, NY. The role involves ensuring members receive necessary care and services, and requires a New York State RN license and ability to travel around downstate New York. This is a full-time position with a flexible work schedule and no weekends, holidays, or night shifts.
Requirements
- New York State RN license
- Ability to travel around downstate New York
- Minimum 1 year of pediatric clinical field experience for PEDS positions
- Previous field-based experience in geriatrics, home care, discharge planning, case management, and/or medical surgical nursing
