Role Overview

Hera is dedicated care management in the community for older adults, covered by Medicare. As a Hero, you'll provide real, ongoing support to families, working autonomously and remotely.

What You Will Do

Serve older adults and their families in aging at home safely, fostering connection and providing care management.

Requirements

• LMSW, LCSW, or RN (not required but a plus)
• Experience in geriatric care management, hospice, palliative care, discharge planning, rehab, or hospital social work
• Ability to foster connection with older adults and their families
• Creative problem-solving skills
• Tech-savviness and ability to leverage AI to supercharge work
