Job Description
The Care Manager II is a lead member of the team responsible for reducing and preventing emergency room visits, hospitalizations, and decreasing Medicaid costs for adults and youth in the community setting. This role involves comprehensive assessments, care planning, and coordination of services to meet client needs, leveraging person-centered and family-driven care strategies. The manager will visit clients with assigned caseloads, provide referrals, and maintain records.
