Support Coordinator (Hospital Discharge) position available at Musgrove Park Hospital and The Link in Yeovil. The successful candidate will work collaboratively with hospitals, housing teams, and other services to support homeless and rough sleeping customers, creating person-centred discharge plans and ensuring a smooth transition from hospital to accommodation.
