Hyacinth AIDS Foundation is seeking a Community Case Manager to provide brief and concise statements explaining the purpose of this position, along with specific duties and responsibilities. The role involves case management, referral services, and collaborating with medical providers and community resources, particularly focusing on newly diagnosed HIV and outreach to clients in North Hudson. A strong emphasis is placed on confidentiality and adherence to HIPPA guidelines.

Requirements
