Community Health Worker

Hyacinth AIDS Foundation is seeking a Community Health Worker to provide crucial services to individuals living with HIV in West New York. This role focuses on outreach, connection to medical services, and providing information to help clients navigate HIV care. The position requires collaboration with a team, adherence toHIPAA guidelines, and representation of the agency’s commitment to HIV research and prevention.

Requirements

• Collaborate with staff in local clinics to outreach to out-of-care individuals and reconnect them to HIV medical services and other supportive services
• Conduct outreach to the hardest to reach, at-risk communities, including men and women with substance use or homelessness
• Refer and link undiagnosed clients to rapid HIV testing services
• Follow up and assist with accurate client record keeping
• Maintain client information in E-Compass and CAREWare
• Attend and participate in staff meetings
• Adhere to Hyacinth policies and procedures
