Job Description
The Hyacinth AIDS Foundation is seeking a Harm Reduction Specialist to perform duties related to the implementation and evaluation of Syringe Access Program-Mercer County. This role involves driving van, maintaining supplies, assessing needs, conducting outreach, and providing education. The specialist will work with individuals at high risk for HIV and other STIs, and will work with substance users in specific communities.
Requirements
- Drive mobile van to specific locations
- Maintain stock of supplies
- Conduct assessments of need
- Document data and participate in audits
- Provide referrals and education
- Identify and assist individuals in need
- Enter data into NJHIV database
- Complete monthly reports
- Update client case records
