Job Description
The Hyacinth AIDS Foundation is seeking a PT Harm Reduction Specialist to implement Syringe Access Program-Mercer County. This role involves driving mobile vans, ensuring a safe environment for injection, conducting assessments, providing education on harm reduction, tracking syringe returns, and maintaining data collection. The position also requires community outreach and collaboration with healthcare providers.
Requirements
- Drive mobile van to specific locations in Mercer County.
- Ensure mobile van is properly stocked with supplies.
- Conduct an assessment of need for injection frequency and number of injections.
- Complete and maintain data collection.
- Conduct outreach to substance users in high-risk communities.
- Provide syringes and supplies.
- Provide information on medication/treatment options.
- Track referrals and outcomes.
- Maintain client case records.
- Prepare monthly reports.
