Job Description
Included Health is seeking a Pediatric Behavioral Health Nurse Care Manager to support children, adolescents, and their families holistically through behavioral health challenges. This role focuses on coordinated, family-centered care, leveraging evidence-based interventions and collaborative teamwork. The ideal candidate will be passionate about their work and committed to ensuring quality care for both short- and long-term outcomes.
Requirements
- Deliver coordinated, family-centered virtual Care Management by phone.
- Develop personalized, goal-oriented care plans.
- Support members through behavioral health treatment plans.
- Navigate behavioral health systems and resources.
- Coordinate resources addressing clinical, behavioral, and social determinants of health.
- Manage volume while maintaining quality of care.
- Comfortable working Monday-Friday 9am-6pm PST
