Job Description
Lumeris is seeking a Population Health Specialist to engage patients, providers, and staff through data-based approaches to implement value-based care workflows. The successful candidate will support population health management efforts, improve provider experience, and positively influence Accountable Care Team behavior.
Requirements
- Bachelor's degree in Healthcare Administration, Business Administration or related field or equivalent
- 1+ years of healthcare experience (i.e., health coach, medical assistant, practice-based role, patient navigation, etc.) or the knowledge, skills, and abilities to succeed in the role
- Ability to operate in a fast-paced and dynamic environment
- Effective communication skills with the ability to interact with multi-disciplinary teams and patients
- Strong organization skills, detail-oriented, and ability to consistently meet and track timelines and goals
