Counselor Case Manager, Sterling Outpatient

Lutheran Social Services of Illinois (LSSI) is hiring a full-time Counselor Case Manager to provide whole-person services to clients participating in Sterling Outpatient programs. The ideal candidate will have a Bachelor's degree in social work, counseling, or a related field and one to three years of clinical experience.

Requirements

• Conduct intake assessments using IM+CANS to evaluate need for mental health services, resources, and plan appropriate treatments
• Provide client-focused, collaborative, and empowering services for clients onsite and in the community
• Maintain a caseload of adequate size necessary to meet identified productivity requirements
