Assisted Living Nurse (RN)

Lyngblomsten is seeking an RN for Assisted Living and Home Care at our Como Park campus. We offer exceptional benefits and great work-life balance.

Requirements

• Conducting resident assessments per policy and regulations
• Supervision of unlicensed staff
• Care plan development and implementation of services to meet resident needs
• Collaboration with physicians, lab, hospice, and other outside providers
• Staff education and training
• Working closely with residents and families to coordinate care
• Assisting with care conferences when needed
• Providing back-up to our after-hours triage service
