
Job description
Join Mercy Health as an Assessment Coordinator to support the Home Care Team. You will be the primary contact for clients of CHSP and other programs, conducting comprehensive assessments and managing client needs.
Conduct assessments, triage clients, identify and manage risks, complete scheduled and reviews, maintain accurate records, and promote Mercy's Healthy Aging services.
You will be a resilient, compassionate, and committed individual with excellent time management, organisational, and communication skills. You will have a good understanding of Home Care funding programs and eligibility.
