Case Manager

The Supported and Safe Integrated Tenancy Program is seeking a Case Manager to work with residents facing complex social and personal challenges. The program provides on-site case coordination and holistic support to individuals and families experiencing issues such as family, domestic and sexual violence, and mental ill health. The role involves developing personalized, culturally appropriate support plans, coordinating referrals to partner services, and building respectful relationships with residents and stakeholders.

Requirements

• Two years of experience in Human Services, a relevant degree, or equivalent qualifications and more substantial relevant experience.
• Demonstrated experience in applying the Risk Assessment Management Framework in a case management or support role.
• Knowledge of issues contributing to homelessness such as family violence, mental ill health, overcrowding, and substance use.
• Cultural competency and experience working respectfully with Aboriginal and Torres Strait Islander peoples, their communities, systems, and protocols.
• Sound communication and engagement skills, with the ability to build trust and collaborate across diverse individuals and services.
• Effective time management, written documentation, and IT proficiency, including case note writing and use of Microsoft Office.
• A resilient, professional approach with a strong sense of boundaries, self-awareness, and commitment to continuous improvement.
