
Job description
We are seeking a compassionate and skilled Physical Therapist to join our team in a skilled nursing facility setting. The Staff Physical Therapist evaluates and treats residents, communicates with families, physicians and other health team members, and maintains documentation of services in the medical records.
Evaluate residents to determine physical therapy needs, develop and implement individualized treatment plans, assist nursing department with training of Restorative Aides, and communicate regularly with physicians and nursing staff.
Must maintain confidentiality of residents' information, report any problems with department equipment, and contribute to quality assurance and performance improvement initiatives.
