After Hours Admission/Visit Registered Nurse (RN)

Pathways Home Health & Hospice is seeking a Registered Nurse (RN) for an After Hours Admission/Visit position. The RN will provide skilled nursing visits to patients and their families, assess and develop individualized plans of care, and coordinate with the Interdisciplinary Group. The role requires a strong nursing assessment, communication, and problem-solving skills, with a focus on holistic, family-centered care.

Requirements

• California RN license
• Graduate of an NLN accredited nursing school
• Minimum of one-year experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing
• California driver’s license and automobile insurance with minimum Bodily Injury Liability coverage Insurance
• Current CPR Certificate– American Heart Association “BLS Health Care Provider”
