Home Health RN Case Manager

The Home Health RN Case Manager provides initial and ongoing assessments of patients, develops and implements plans of care, and coordinates home health care services. Provides prenatal and after-pregnancy care, supports case management improvement, and participates in orientation and competency testing.

Requirements

• Valid California driver's license and dependable insured automobile
• Ability to travel within the PPCCCHH designated geographic area
• One year of professional nursing experience, postpartum experience preferred
• Graduate of an accredited school of nursing
• Current active RN license issued by the California Board of Registered Nursing
• BLS certification
• One year of experience with lactation support
• Proficiency with PMADs assessment
• English-Spanish bilingual skills
