Resident Care Coordinator – Senior Living

We are seeking a Resident Care Coordinator to provide clinical and operations support to seniors in various settings, including senior living and individual homes. The successful candidate will work closely with an RN and provider to drive patient outcomes and foster positive relationships with patients and their caregivers.

Requirements

• High School Diploma / GED (or higher)
• 2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support
• Medical Assistant, Patient Care Technician, or equivalent certification preferred, otherwise 5+ years of healthcare experience
• Experience in geriatric care or senior living community is highly desirable
• Must possess a valid and current driver's license
